1. Eligibility Based on BMI (Body Mass Index)
- BMI of 35 or higher
→ Eligible for insurance coverage regardless of other conditions. - BMI between 30 and 35 with at least one obesity-related comorbidity, such as:
- Type 2 diabetes
- High blood pressure
- Sleep apnea
- High cholesterol
- Non-alcoholic fatty liver disease
→ Eligible for coverage if any condition is present.
2. Age Requirements
- Generally available for patients 18 years and older.
- Pediatric and adolescent patients may be considered on a case-by-case basis.
